We at Surbiton Smile Centre would like to thank you for choosing our practice. For your benefit and convenience we are proud to be able to offer you Dental Care Plan-our membership scheme, which enables us to provide you with the treatments and support necessary to control dental disease and restore your mouth to full fitness.

Dental Care Plan £14.50 per month.

Regular dental appointments (two per year) to provide: -Clinical examination
-Checking for signs of oral cancer
-X-rays where clinically necessary (up to two small x-rays per year).
Regular hygiene appointments (two per year) to provide: -Scale and polish
-Periodontal advice
-Preventive techniques to reduce decay and gum disease.
Assessment of emergencies and dental pain with any temporary treatment required, carried out at the practice during normal surgery hours.
All treatment planning for your future dental needs.
15% discount off any laboratory-based treatments required, i.e. crowns, bridges and dentures.15% reduction off any fillings.
15% discount off other dental work (extractions, etc.).*
15% discount off any additional hygiene work.
Worldwide Trauma Insurance to protect against the cost of large unforeseen accidental damage.
Emergency Callout Insurance, should you need a dentist in an emergency, anywhere in the world.
Redundancy protection for up to 12 months.**

Please note, discounts only apply to treatments carried out at the practice.

* Excluding implants and sedation. Patients using our finance facility do not qualify for 15% discount.
** This only applies to the paying patient, and does not cover any spouse or dependents.

If you would like to register for the plan please call in at the practice where an agreement will be completed, or we will be happy to post an agreement form to you.

Bear in mind that your first dental consultation will not be part of the plan.

The agreement will require your signature and bank account details for the Direct Debit Mandate. Direct Debits will be collected on the 8th of each month.
